Do I Need a Fit Note?
A fit note, sometimes called a sick note, provides medical evidence about how your health affects your ability to work.
Off Work for 7 Days or Less?
You do not usually need a fit note from your GP if you have been off work for 7 calendar days or less, including weekends and bank holidays.
You can self-certify your absence from work. Your employer can tell you how they would like you to do this.
Off Work for More Than 7 Days?
If your health condition means you have been unable to work for more than 7 calendar days, you may need a fit note.
You can submit a request to Southend Coastal Surgeries through Anima.
Please provide as much information as possible, including:

The health condition affecting your ability to work

When you stopped working
-
How your condition is affecting you
-
How long you expect to be unable to work

Whether you need a new fit note or an extension
Extending an Existing Fit Note
If your current fit note is coming to an end and you are still unable to return to work, you can request an extension through Anima.
Please include the details of your previous fit note and explain how your condition is currently affecting you.
A clinician will review your request and may need to speak with you before another fit note can be issued.
Can a Fit Note Be Backdated?
In some circumstances, a fit note can cover an earlier period if a healthcare professional is satisfied that your health condition affected your ability to work during that time.
This will need to be clinically assessed and cannot be guaranteed simply because a backdated note has been requested.
You May Be Fit for Work With Support
A fit note does not always mean you need to remain completely off work.
A healthcare professional may advise that you may be fit for work if your employer can make appropriate adjustments, such as:

A phased return to work

Different working hours
-
Amended duties
-
Changes to your workplace
You can discuss these recommendations with your employer.
Fit Notes Are Free
There is no charge for an NHS fit note when you have been off work due to illness for more than 7 days.
