Learning Disability Annual Health Checks

 

Your Annual Health Check

If you are aged 14 or over and on your GP practice’s learning disability register, you can have a free Learning Disability Annual Health Check every year.

​

The appointment gives you extra time with our healthcare team to talk about your health, wellbeing and any support you may need.

 

Why Is Your Health Check Important?

People with learning disabilities can sometimes experience health problems that are difficult to recognise or communicate.

Your annual health check helps us:

  • Check your general health and wellbeing

  • Identify possible health problems early

  • Review your medication

  • Check that vaccinations and screening are up to date

  • Talk about any changes or concerns

  • Make sure you are receiving the right support

  • Discuss any reasonable adjustments that could make accessing healthcare easier

 

You do not need to feel unwell to have your health check. It is about helping you stay healthy and well.

Your Appointment

We want your appointment to feel comfortable and unhurried.

Appointments are usually around 40 minutes, giving you plenty of time to talk to our team and ask questions.

Where possible, we can arrange the appointment at a time that works well for you.

You are welcome to bring a family member, carer or someone you trust to support you.

Before Your Appointment

Before your health check, we may send you or your carer a questionnaire to complete.

Please complete it before your appointment and bring it with you. This gives our team useful information about your health and helps the appointment run smoothly.

Don't worry if you need help completing it — a family member, carer or our team can support you.

​

Supporting You in the Community

Your health is about more than what happens inside the GP surgery.

​

Our team can help connect you with local health, wellbeing and community services that may support your independence, wellbeing and everyday life.

​

This could include support with healthy lifestyles, social activities, mental wellbeing, caring needs and other local services.

Reasonable Adjustments

Please tell us if there is anything we can do to make your visit easier.

This could include extra time, a quieter waiting area, easier-to-understand information, support with communication or arranging your appointment at a suitable time.

We want you to feel safe, comfortable and listened to throughout your care.
