Medication Reviews
Keeping your medication safe and effective
If you take regular medication, you may be asked to have a medication review from time to time. This helps us make sure your medicines are still appropriate, effective and safe for you.
During a medication review, a member of our clinical team may check:
-
Whether your medication is working well for you
-
Any side effects or problems you may be experiencing
-
Whether your dose or medication needs changing
-
Whether you are still taking all the medicines on your repeat list
-
Whether any medicines are no longer needed
-
Whether you need blood tests, blood pressure checks or other monitoring
What happens when my medication review is due?
We may contact you by text message or telephone.
Depending on your medication, you may be asked to complete a questionnaire, provide readings, have a blood test or speak with a member of our clinical pharmacy or medical team.
Please complete any requested monitoring promptly. In some circumstances, we may not be able to continue issuing medication safely without an up-to-date review or the required monitoring.
